October 1, 2015 ICD-10 Implementation and Compliance Guideline: a. For On/After October 1, 2015 date of service (DOS) Use ICD-10 code on both PM160/CHDP and CMS-1500 (HCFA) forms. b. Before October 1, 2015 date of service (DOS) use ICD-9 code on both PM160/CHDP and CMS-1500 (HCFA) form.
January 1, 2014 Effective January 1, 2014 Anthem Blue Cross will make available electronic submission of the Information-Only PM 160 form.
